在中国，基本药物政策已经存在差不多20年了，却始终无力有效运行。在国家医疗卫生改革方案中，重提这一概念是一个很大的进步。本文讨论了几个需要克服的结构性障碍，其中最主要的问题就是供方支付体系。基于最近的改革经验，人们已经讨论到了改革基本药物体系需要注意的几个关键问题，其中包括试点本身的价值、如何选择基本药物、集中采购、零差价政策和医药分离等等。仿制药政策将刺激市场提供高质量、低成本的仿制药，从而从总体上提高供给能力，同时又保证药品价廉物美。本文最后前瞻性地介绍了要想成功实施以低价仿制药品为支撑的基本药物政策所必需的一些关键因素。
